Enrolment Process

At Lakelands Primary School, enrolling your child is a straightforward four-step process.


Stage 1 – Download and Complete Application Forms

Enrolments

  • Application to Enrol Part A  
  • Form 1: Student Health Care Summary – required for all new enrolments

Required Supporting Documents:
Please submit completed application documents along with:

  1. Birth Certificate
  2. Medicare Immunisation Record – must state UP TO DATE
  3. Proof of Address – utility bill, drivers license, lease agreement or rates notice

You may submit documents in person or via email: Lakelands.PS@education.wa.edu.au
(Please note: Email attachments must not exceed 10MB.)


Stage 2 – Meeting with the Principal or Deputy Principal

The next step is an enrolment interview with our Principal or Deputy Principal.
We encourage families to bring their children along so they can meet the Principal and become familiar with the school environment.

After the interview, you will receive an Enrolment Pack containing:

  • Enrolment Form – Part B

  • School information

  • Book list

  • Uniform list

  • School Voluntary Contributions and Charges
  • Other relevant documents to help you prepare for your child’s start at Lakelands Primary School


Stage 3 – Finalise Enrolment

Once your enrolment interview is complete:

    1. Return the completed Enrolment Form to the school office via email or in-person.

    2. Order your child’s School Uniform – LakelandsPS_2026 Pricelist– Perm-a-Pleat Mandurah

    3. Purchase Personal Items (books, stationery, etc.) – Personal Items List | Lakelands Primary School


Stage 4 – Start School

The final stage of the enrolment process is your child’s first day at Lakelands Primary School.

  • On their first day, your child will be welcomed by our principal in the front office and walked to class to introduce them to their new teacher and peers.

Starting school is an exciting milestone, and we are here to make the transition smooth and enjoyable for both you and your child.
